By providing your phone number, you consent to receive Application-to-Person (A2P) messages from Team Mentation, PLLC. These may include:

-Appointment reminders

-Prescription updates

-Billing and account notifications

-Practice announcements and other service-related messages

Opt-In: Consent is collected at the time you provide your phone number, either through intake forms, online submissions, or verbal agreement.

Opt-Out: You may opt out at any time by replying STOP to any message. For assistance, reply HELP or contact us directly.

Message & Data Rates: Standard carrier rates may apply. Frequency of messages will vary depending on your care plan and service needs.

5. User Responsibilities

Team Mentation, PLLC is not responsible for:

-Delays or failures in message delivery caused by carriers or technical issues.

-Charges incurred by your mobile provider for SMS/MMS communications.

-Any damages arising from your misuse of our services.

We may update these Terms at any time. Updates will be effective immediately upon posting to our website. Continued use of our services constitutes acceptance of the revised Terms.
